Declaration made this 15th day of July 2010*, I Floyd Jay Winters willfully and voluntarily make known my desire that my dying not be artificially prolonged under the circumstances set forth below. Under no circumstances do I wish the members of the Legislature to enact a special law to keep me on life-support machinery. Under no circumstances do I wish any governor of Florida to violate my rights as a human being and order a doctor to force a feeding tube down my throat against my will or against the will of my designated guardian and wife, Julie Manchester.

 

I do hereby declare that, if at any time I am incapacitated and

(initial) I have an end stage condition _FJW______

(initial) or I am in a persistent vegetative state _FJW______ 

 

Then the discussion about terminating my medical treatment should remain private and confidential and shall involve my wife, Julie Manchester, and medical experts, not grandstanding, hypocritical politicians who would joyfully allow me to suffer in lingering, excruciating, agony if it could garner a few more fundamentalist votes. I also grant my wife full power of attorney if I am unable to make important decisions concerning my medical treatment.

Give me a reasonable time and chance to recover if it is logically and physically possible for me to recuperate. But I do not want to end my life in prolonged, terrible pain or in an undignified and badly crippled state that I cannot possibly recover from. If my wife agrees with the attending or treating physician and another consulting physician who have determined that there is no reasonable medical probability of my recovery from such condition, I direct that life-prolonging procedures be withheld or withdrawn when such procedures would serve only to artificially prolong the process of dying, and that I be permitted to die naturally with only the administration of medication or any medical procedure deemed necessary to provide me with comfort or to alleviate pain.

If I am old and infirm and I choose not to eat, do not force food into my mouth or sustenance into my veins so that some corporate entity can make money, or some relative or pastor can say that it is Godís will that they needlessly prolong my suffering.

If I am old and firm and willing - please can contact my wife, assuming she is still somewhat firm, and give us a little privacy.

I wish to peacefully meet my maker when he calls me, not after the religious far-right has made a mockery of my life and death. History has repeatedly taught us, whether Christian, Jew, or Muslim - the religious right is neither, and my wish is that absolutely no member of the religious far-right have any involvement in my final days. It is my intention that this declaration be honored by my family and physician as the final expression of my legal right to refuse medical or surgical treatment and to accept the consequences of such refusal. 

In the event that I have been determined to be unable to provide express and informed consent regarding the withholding, withdrawal, or continuation of life-prolonging procedures, I wish to designate, as my surrogate to carry out the provisions of this declaration the following person:

Additional Comments: If any of my family goes against my wishes and turns my case into a political cause, may the religious far-right visit their marital bedroom.
